Major contraindications include hypersensitivity to Tramadol and opioids, gastrointestinal obstruction, intoxication with alcohol and hypnotics, concomitant use of central nervous system depressants, psychotropic drugs, monoamine oxidase inhibitors, patients with severe hepatic or renal impairment, acute asthma, airway obstruction, status asthmaticus, and acute respiratory depression.
During recent years some opioids have been associated with prolonged QT and torsade de pointes TdP. This indicates that QT prolongation and TdP could be a more general problem associated with the use of these drugs. Comparative data from studies were not available for Zytram XL and Ralivia. This aspect of Durela may help in providing the chronic pain patient with adequate plasma concentrations of Tramadol throughout the hour dosing cycle, as well as minimizing the risk of breakthrough pain and side effects.

Pagets disease is a disorder of bone turnover of unknown aetiology producing characteristic bone deformities.
