Patients must not be taking other photosensitizing drugs, for example phenothiazines and benzodiazepines, NSAIDs and some antibiotics. Psoralens are naturally occurring plant extracts found in the parsnip family and the technique has a long history, being used by Egyptians and Indians to treat vitiligo as long ago as in the skin and photochemotherapy is still used to treat it. Psoralen causes stimulation of melanocytes and some repigmentation in vitiligo.
Paracetamol is therefore not of value in chronic rheumatic diseases, or in situations where the pain is due to inflammation. When taken at the recommended dosage, there are few side effects with paracetamol and no major interactions with other drugs. In substantial overdose, severe, life-threatening liver damage is likely to occur due to excessive production of a toxic metabolite. There is an antidote to paracetamol overdose (acetylcysteine) and provided it is given within eight hours a complete recovery can be made. In the liver, acetylcysteine is converted to glutathione, which forms a harmless conjugate with paracetamol.
Aspirin used to be the obvious first choice anti-inflammatory analgesic, but its use has been superseded by less the toxic drugs. Aspirin is more likely to be used as a short-term analgesic than an anti-inflammatory drug. Ibuprofen, fenbrufen and naproxen are now the drugs of first choice because they are the least likely to cause gastric irritation and ulceration, a common side effect of all NSAIDs. This is because another role of prostaglandins is to reduce acid secretion in the stomach. Inhibition of prostaglandin synthesis leads to excessive acid secretion and inflammation of the gastric mucosa and duodenal mucosa.

COX have been developed which are selective COX likely to cause the gastrointestinal side effects. Polyethylene glycol is an osmotic laxative used for bowel cleansing (see below). Although only small amounts of these drugs are absorbed, osmotic laxatives should not be used in patients with poor renal function because of the risk of magnesium accumulation. High concentrations of magnesium in the body can interfere with the function of calcium ions in the heart, skeletal muscle and the central nervous system.
The only exception to this rule is when a patient must always receive the same brand of a drug because different preparations can result in different blood levels of the drug. No details of dosages are given in this book, because these are subject to change and often have to be varied to suit individual patients. In practice, the BNF or MIMS should be used as a guide to dosages. Examples of individual drugs have been kept to a minimum in the text, with usually just one or two examples given in each section. It would be impractical to try to remember the names of all drugs available.
Supplementary prescribers have few restrictions on the medicines that they can prescribe. As long as the medicines can be prescribed by a doctor at NHS expense and are referred to in the patients CMP, they can be prescribed. Supplementary prescribers prescribe in partnership with a doctor.
